Shone’s syndrome is also known as shone complex. It is a rare disease with less than 1% of all the congenital heart disease.  

Shone’s syndrome require surgical intervention in all cases. The patient may undergo many surgeries initially and the surgery may repeat throughout the life.

Shone complex or shone’s syndrome is a defect in the heart, present at birth. There are 3 or more defects that affects the blood flow in the left part of the heart. The blood circulation from the left part of the heart is blocked in multiple sites. The 8 heart conditions linked with Shone’s syndrome are –

The objective of the treatment is to correct the restricted blood from the left side of the heart. The lesions that causes blockage are removed surgically. 

The surgical treatment of Shone’s syndrome treats the following heart defects –

1. Coarctation of the aorta (narrowing the part of aorta) – Prostaglandin may be used in severe narrowing of aorta in newborns. Usually two types of surgery are performed -

  • Excision with end to end anastomosis
  • Subclavian flap angioplasty

In older children, where surgical repair is done followed by recoarctation, the treatment of choice is transcatheter balloon angioplasty. In this a hollow tube is inserted at the site of blockage in aorta and a balloon is places to expand the vessels.

2. Subaortic stenosis – when there is obstruction in the blood flow from left ventricle into the aorta it is surgically treated by the method septal myectomy (removal of excess tissues in the aortic valve). Depending upon the type of aortic stenosis, the doctor may recommend aortic valve replacement.

3. Mitral stenosis – it may be corrected by valve repair or valve replacement.

The outcome of the treatment depends on the severity of the disease and the overall effect of the surgery. If there is valve replacement surgery, then most of it last 10-20 years.
